Dental and Vision Plan Coverage is now available, and "open season" ends December 11. This coverage requires additional payment, of course. Crew Supe Robert Yeager said "Did some research on the plans and we like met life high option, costs $38.11 per month. This is for dental only. For vision we like blue cross high option - you can change your plans next open season if you don't like your plan."

| The Baseline Exam includes a PPD skin test which requires the examinee to return within 48-72 hrs. for reading. Also, there is a 'resting' EKG in the Baseline (for ALL aged employees)
The blood tests for a Baseline Exam include the following: Blood Chemistry, CBC & Lipids along with a Urinalysis (no drug screen). The Chemistry includes cholesterol and various proteins. The CBC includes iron count. The Lipids includes the breakdown of cholesterol (good and bad). The Urinalysis includes blood glucose (sugar) levels. Depending on any clearance requested (if any), some exams include your blood type, lead and zinc levels. Exam results can be sent to examinees who submit a written request. If someone is interested in all or part of their exam results, have them fax to (703) 288-5482 a request in writing stating their full name, social security number, and what they would like to receive. They must have their signature and a return address. STEP BY STEP PROCESS: 1. Everyone should have received the 13-page packet by now. If not, contact Steve Theisen. 2. Comprehensive Health Services is in the process of calling folks and scheduling appointments according to our zip codes. They are naturally not as concerned about our deadlines as we the jumpers are, so if you haven't heard from them by now, or if you are in another geographic area and would like to reschedule your exam, call them at (800) 638-8083. Ask for Christine at extension 11606. She is much more helpful than the other name also listed on our contact sheet there. 3. Fill in the shaded portions of the 13-page packet and take it with you to the doctors office. You will leave the completed packet with the doctor and they will mail it. FAST 12 HOURS PRIOR TO YOUR EXAM. (DON'T EAT.) Take your glasses or contacts if you need them to pass the vision test. 4. Remember, the sooner you take the exam, the sooner you can re-take it if there are any problems - for example, high blood pressure etc. Scheduling exams at refresher time in Fairbanks in April could be a cluster, and word has it that you will be 'off the list' until it is completed.
